You can apply for the Financial Assistance Program after your test is complete or when you receive your bill. If approved, assistance may reduce your final out-of-pocket cost.

Step 2: Submit your application (PDF only)
If you completed a PDF form, email your signed application to [email protected]. We accept electronically filled PDFs, scanned copies, and photos.

We will contact you regarding approval within 2–3 business days. To avoid delays, please complete all fields. Additional information may be required.
