What is required for ordering GenomeDx testing?

  - Completed GenomeDx (oligo aCGH) Submission Form, including Payment Options Form or Institutional Billing Instructions and informed consent .
  
  Please note that an accurate and specific interpretation of test results can only be reported if relevant clinical information is provided to us.



• Specimen Requirements
  
  - Proband: 1-3 mL whole blood in lavender top (EDTA) tube
  
  - Biological parents (strongly recommended): 3-5 mL whole blood in lavender top (EDTA)
  
  Please note that testing of the proband's parents may provide essential information (de novo vs. familial alterations) for accurate and rapid interpretation of the proband's test results.
  
  Analysis of parental samples is performed at no additional charge.


Laboratory clients may submit high quality genomic DNA specimens prepared in a CLIA-approved laboratory. The total amount should exceed 5 micrograms of DNA, with a concentration of at least 50 nanograms per microliter (0.05 micrograms per microliter). Please state the concentration sent.

Occasionally follow-up FISH testing on parental samples may be requested by GeneDx. If indicated, please submit 3-5 ml whole blood in a green top tube (Sodium Heparin) for each parent.
