Financial Policy

• GeneDx does not bill commercial insurance directly except when all of the following criteria are met:
  
  1)   A written letter of pre-authorization accompanies (or preceeds) the sample. This letter should state that the request for genetic testing for the specific disease/gene has been approved. It is most helpful if the level of reimbursement is also included in the letter. In most cases, an authorization number alone will not be acceptable.
  
  2)   A photocopy of the front and back of the patient's insurance card must accompany the sample, and the address to which the claim is to be submitted must be supplied, as well as a contact phone number and name.
  
  3)   As the patient will be responsible for any costs of testing not covered by the insurer, GeneDx requires that a credit card be provided to which the balance of the fees, after insurance reimbursement, will be charged. Complete the payment options form with the credit card information and submit the form with the specimen. The patient will be notified prior to any charge being made against the card.
